THE USE OF ALTERNATIVE MATERIALS IN A TOXICOLOGICAL EXPERT ANALYSIS BY THE LC/APCI/MS METHOD FOR THE EVALUATION OF A COMPLEX LETHAL POISONING WITH MEDICINES: CLOMIPRAMINE, TIANEPTINE AND HYDROXYZINE

Streszczenie
In the current work the authors present the case of a suicidal poisoning of a 21-year-old women A. B. with clomipramine, tianeptine and hydroxyzine. The toxicological expert study executed with the HPLC/APCI/MS method, besides standard materials used in classic toxicological expert studies such as blood, urine, the liver or the kidney, also encompassed alternative materials, such as body fluids, vitreous humour, labyrinthine fluid, the cerebrospinal fluid and hair. Analysis showed the presence of medicines in the classical post-mortem material in concentrations explaining the deadly poisoning with clomipramine, tianeptine and hydroxyzine. Analysis of the content of these medicines and of their metabolites enabled us to reveal differences in terms of presence of metabolites in particular materials. The presence of medicines in 12 cm long segments of hair confirmed the fact known from an interview that A. B. was treated for depression during the last year with the medicines revealed in the course of the expert analysis, which she finally took in a toxic dose with suicidal intention.
